Canadian Immigration Physicals : Screening examinations required by the Canadian government for any person desiring to emigrate to Canada are processed through the eMedical system. An appointment must be made with us NO WALK-INS. Please go to our PATIENT FORMS tab on this website to access the "HIPPA Contact Preference Form" that you will need to fill out and bring to your visit with us. This is the only specific Quality Internal Medicine form you will need to fill out. However, there are additional immigration forms that are required that will be filled out at the time of your visit so it is important that you arrive promptly at your appointment time to make sure you have adequate time to fill out the forms before seeing the physician

United States Immigration Physicals: Dr. Eapen is a designated U.S. Civil Surgeon able to perform US Immigration Medical Examinations. QIM has all the forms available in the office to complete these exams. You can also click here to fill out and/or print and bring the required medical history form with you to your visit.
