Canadian and Australian Immigration Physicals: Screening examinations required by the Canadian and Australian governments for any person desiring to emigrate to Canada or Australia. All Canadian and Australian immigration forms are available at our office but cannot be printed or accessed online. Please go to our PATIENT FORMS tab on this website to access the "HIPPA Contact Preference Form" that you will need to fill out for your visit with us - this is the only Quality Internal Medicine form you will need to fill out in addition to the Immigration Forms.

United States Immigration Physicals: Dr. Eapen is a designated U.S. Civil Surgeon able to perform US Immigration Medical Examinations. QIM has all the forms available in the office to complete these exams.
