You must obtain and inform our office of your HAP ID number which you will receive when you launch your application electronically. Please go to the following website to start your application process; . The following website explains the different visa classes:

  • The health requirements and thus the type of examination you will need from our office will be determined by the results of your electronic application. A physical examination and/or chest x-ray and/or an HIV blood test may be required. If needed, the chest x-ray will be done at a radiologic facility in the building next door to our office.
  • If you have difficulty reading or speaking English, please bring an interpreter to your appointment whenever if possible.
  • 3 color passport size photos are needed PER PERSON. A PASSPORT is required for identification verification (a driver's license is NOT acceptable).
  • If you were given forms 160 (Radiology Form) and 26 (Medical Form), please bring these to your appointment. If you do not have the forms, we can provide them.
  • You DO NOT need to fast and please come fully hydrated for possible blood work and urine testing. Based on an individualís history, additional blood work over an above the routine
  • HIV testing may be necessary and these would need to be performed for an additional fee above the routine exam fees.

If you are taking any prescription medications, you MUST bring in the medication bottles. If you have any chronic medical conditions, there is a possiblity that we may need to get records re: the status of these conditions from your physician

For females: If you are having your menstrual period at the time of your appointment, please use tampons on the day of your appointment to prevent menstrual blood from contaminating the required urine test.
